1. Who we are

FormIQ is operated by Ganda Tech Services Pty Ltd, a technology company based in New South Wales, Australia.

2. What data we collect

When you generate a form, FormIQ sends the following to our API:

• Your selected specialty (e.g. psychology, physiotherapy)
• Any custom instructions you enter (e.g. "include a K10 scale")
• Your account identifier (used to enforce plan limits)

No patient data is ever sent to the API. FormIQ generates form templates only — it never receives, stores, or processes patient information.

3. How we use your data

• To generate the intake form you requested
• To enforce plan usage limits (free vs Pro)
• To improve the accuracy of generated forms over time

We do not sell, share, or disclose your data to third parties except as required to operate the service.

4. Where your data is processed

All API requests are processed on Google Cloud Run in the australia-southeast1 (Sydney) region. Data does not leave Australia.

5. Data retention

Generated form schemas are stored against your account until you delete them or close your account. Usage logs are retained for 90 days.

6. Your rights

Under the Australian Privacy Act 1988, you have the right to access, correct, or request deletion of your data. Email support@g-t-s.com.au and we will respond within 30 days.

7. Cookies

The FormIQ WordPress plugin does not set cookies. The FormIQ web app uses a session cookie solely for authentication — not for tracking or advertising.

8. Changes to this policy

Material changes will be notified by email to registered users. Continued use after notice constitutes acceptance.
